Medical Director, oversight, review and pre-admission medical clearance
leadership
Tulum,
An oceanfront plant-medicine centre in Tulum selling ibogaine and ceremony work at three-hundred-thousand-dollar tiers, with one clinician named.
Sanctuary Tulum publishes a rate card that starts at USD 300,000 and runs past USD 1,000,000, and it names exactly one clinician for the whole operation. It is the Caribbean property of the Holistic Sanctuary group, built around the founder's proprietary Pouyan Method: no prescription medication, no group therapy and, in the site's own framing, no talk therapy. Care is organised around medically supervised ibogaine, ayahuasca, psilocybin and 5-MeO-DMT ceremonies, wrapped in a daily stack of intravenous, regenerative and biohacking interventions. Three to five clients are in the building at a time, each in a private suite, with the Platinum tier taking the building down to one. What structurally distinguishes it is the price point set against the published clinical roster, and the fact that a sister property under the same method and the same medical director sells four weeks for roughly one sixth of the entry tier. The group site lists Tulum and Rosarito as its two locations under a common owner; Sanctuary Tulum's own material names no affiliated brand, no parent company and no sister property.
One clinician is named in the entire organisation. Nurses, emergency physicians, facilitators and coaches appear only as unnamed headcounts.

No registration number, cedula profesional, specialty, board certification, medical school or institutional affiliation is published for the Medical Director, and no corroborating professional or registry record was located.
Four to twelve weeks
USD 300,000
The entry tier. A private master suite in a building holding at most five clients, with daily regenerative and intravenous protocols, plant-medicine ceremonies under the published cardiac screening, the proprietary Brain Repair IV protocol, hyperbaric oxygen, infrared sauna and cold plunge, bodywork, breathwork and yoga, organic chef-prepared nutrition, and integration windows of 24 to 48 hours after each ceremony. One to three hours a week of dedicated one-to-one coaching is published for this tier.
Four to twelve weeks
USD 500,000
The oceanfront tier, which the clinic says takes the building down to a single client. Everything in the Gold Package plus oceanfront private accommodation, a deeper regenerative stack including stem cell and ozone work, and a higher ceremony cadence. The clinical difference against Gold is not itemised anywhere: the published difference is accommodation, exclusivity and protocol depth.
Twelve to sixteen weeks
USD 1,000,000 and upward
A private beachfront mansion for one principal, with a private chef, a live-in mental health coach, dedicated fitness and wellness coaching, exosome and stem cell therapy, extracorporeal blood oxygenation and ozonation, plasmapheresis and private ceremonies. Positioned for people who require sole occupancy of a separate property rather than a private suite in a shared building.
Delivered inside the residential packages
Included in the package fee, no separate price published
Pharmaceutical-grade ibogaine hydrochloride rather than plant extract. Published screening: a full EKG, echocardiogram and cardiac risk assessment before administration, complete bloodwork with liver-function and metabolic panels, a medication interaction review, and supervised transition off benzodiazepines or opioids. Published monitoring: continuous cardiac monitoring through the acute phase, on-site physicians and nurses, a crash cart, a defibrillator and pre-arranged hospital transfer. Published exclusions are named rather than gestured at: untreated cardiac conditions, severe liver disease, active suicidal intent and medical contraindication to travel. The protocol is self-published and unaudited. Dose calculation, QTc thresholds, the monitoring parameters, the complete exclusion list, and the name of the receiving hospital and the transfer time to it are Not published, and Tulum has no tertiary hospital.

No independent review base is tied to this facility. No Google Business Profile was located and no Trustpilot profile exists. The substantial review base in circulation, 53 reviews at 4.9 stars on Yelp, is attached to the group's US mailing address in Beverly Hills under a different brand: it is not a listing for the Tulum facility and should not be read as Tulum review data. The group's own page acknowledges further reviews sitting in that platform's filtered section. Authenticity is contested, with both positions on the record. Salon reported on 9 September 2016 that the group's reviews used suspiciously similar language and that negative reviewers were dismissed by the operator as frauds. Against that, the group states it declines pay-to-play directory listings, does not litigate against reviewers, does not manipulate reviews, and invites prospective clients to read the critical ones. Two critical reviews on a consumer complaints platform were removed on 14 February 2018 under that platform's legitimacy verification programme. No lawsuit, court judgment, regulatory enforcement action, licence suspension or documented death in treatment was located, and the Better Business Bureau file records no complaints. Mexican health-regulator enforcement records are not systematically searchable, so the absence of a public finding is not proof that none exists.

Salon published a critical investigative feature on 9 September 2016 about the group's Rosarito property. The Fix published a largely positive client-survey feature on 20 April 2017. Everything else located is press-release distribution or sponsored placement. The homepage carries an As Featured In strip of six outlet logos, unlinked, for which no matching article was found.
None claimed and none found.
A stated 10,000 square foot private oceanfront estate on the Caribbean coast, holding at most three to five clients. Accommodation is private luxury suites and bungalows, never shared, and the clinic states there are no group sessions, no shared spaces with strangers and no institutional atmosphere. Grounds are described with swimming pools, a yoga deck, a massage terrace and a gym facing the ocean, with direct beach access. On-site medical and biohacking infrastructure includes a hyperbaric chamber, red-light panels, an infrared sauna, a cold plunge and an intravenous suite. No street address, no room count, no floor plan and no independent photography exist. The only street-level address the clinic publishes is a business centre on Avenida Kukulkan, and the only address in third-party circulation places the operation at Aldea Zama, an inland residential development roughly 3 km from the shoreline, which does not reconcile with an oceanfront estate. Neither can be confirmed.
A private chef prepares all meals. The published model is 100 percent organic superfood nutrition, cold-pressed juicing and targeted supplementation tailored to the treatment phase, on an anti-inflammatory framework built on bone broth, spirulina and colostrum. Ceremony-day fasting and preparation diets form part of the ibogaine protocol, which is a genuine clinical integration of the kitchen rather than a hospitality flourish. A private chef is an inclusion of the Elite package. No chef is named. No nutritionist or dietitian is named. No dietary, micronutrient or food-sensitivity testing is described, and no kitchen hygiene certification, food-safety registration or culinary credential is published. The nutrition programme reads as central to the model and nothing about its execution is independently verifiable.
single occupancy per residence
On the Gold Package three to five clients are in the same building at once. Each has a private suite, there are no group sessions, and the clinic states there are no shared spaces with strangers. A shared building with shared grounds, pool, gym and yoga deck cannot support a no-contact guarantee, and the clinic does not claim separate entrances, duplicated communal space, dedicated per-client teams or individual transport. The entry tier is therefore single occupancy per residence, not no client contact. At the Platinum tier the clinic states the package takes the building down to a single client, and the Elite tier places one principal in a separate private beachfront mansion. Both are genuine sole-occupancy offers, self-declared and unaudited. Cutting the other way, and this is unusual in the sector: the group publishes filmed client video testimonials on its own website and channel, drives clients to leave named public reviews, and the founder has posted a public response to a critical review on a consumer platform. No confidentiality policy beyond the privacy notice, no staff agreement, no device policy and no media policy is published anywhere. One mechanism beyond statute is published: health information submitted through the clinic's forms is restricted to admissions and clinical staff with access limited to authorised personnel.
Headline basis: Public evidenceThe headline number is the public score, not the extended score., LROAS 1.0.0, edition 2026-10-01
Public-evidence total: 37
The sum of the ten criterion scores, from 10 to 100, using evidence anyone can check, including the clinic's own pages as self-declared statements. For the licence element of C2 only, it can also use an authority ruling held in our sealed evidence room. It is the only number used for the rank.
ConfidenceHow well the evidence supports the score.: low. ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: C10.
Every session is delivered one to one with no group therapy and no shared treatment rooms, and at the entry Gold tier clients have private suites but share one building, which places the standard offer in band 7. The Platinum tier sells sole occupancy of the building, so the one-band tier lift applies and gives band 8, the ceiling for a lifted tier. The lift is scored only at the level the evidence supports: arrival separation, individual transport and scheduling of shared spaces are not established. The clinic's statements about the model agree across pages, so no capacity deduction applies.
ConfidenceHow well the evidence supports the score.: medium
A physician Medical Director holds a stated oversight and clearance role, supervised tapers are provided on site, and nursing runs 24 hours a day with emergency physicians rotating on treatment days. The clinic publishes COFEPRIS licence number 17-AM-02-005-0003, but no register entry for that number could be retrieved, so the licensing element is not met and the licence ceiling applies. No psychiatrist care, overnight physician presence, named receiving hospital, on-site non-psychiatric specialty service, complex-presentation programme or accreditation on the CARF or JCI registers is established, which keeps the clinic at band 3.
ConfidenceHow well the evidence supports the score.: medium
Three assessment elements are published: the Medical Director reviews the cardiology workup, stress test, EKG, imaging and bloodwork and clears or declines each client before admission; pre-admission testing includes metabolic, lipid, blood count, liver and HbA1c panels; and a review call decides suitability. These give band 5. No psychiatric assessment, physician examination on admission or written formulation is published, so the higher bands are not reached.
ConfidenceHow well the evidence supports the score.: medium
Individual work is offered, but the published figure of nine hours a day of one-to-one clinical time is not attributed to any clinician, and the daily regimen consists mainly of infusions, hyperbaric oxygen, sauna, bodywork and coaching, which do not count as psychotherapy. No established psychotherapy modality and no named therapist, counsellor or psychiatrist are published, and the team is pooled. Individual therapy without a counted clinician gives band 3.
ConfidenceHow well the evidence supports the score.: low
Three aftercare elements are published: a written integration plan at discharge, structured check-ins with the clinical team, and help arranging continued support locally. These give band 4. No duration, price, statement of inclusion in the fee, format, psychiatric follow-up or family involvement after discharge is published.
ConfidenceHow well the evidence supports the score.: medium
Two violations are found. A post on the clinic's own blog, bylined by the founder and titled 10 Best Luxury Rehabs in the World, ranks the clinic first, and the homepage carries an As Featured In strip of six outlet logos for which no matching article was found. The llms.txt file lists only navigation and services, so no AI-directed violation applies. Fixed package prices and a stated policy of directing contraindicated applicants to acute care are positive signals but cannot lift an unclean sweep; two violations give band 2.
ConfidenceHow well the evidence supports the score.: low
Two elements are met: a licence number is published on the clinic's own site, and the clinic states plainly what the programme does not do, including that it cures nothing and has not been through randomised controlled testing. No exclusions, payment, refund, extension or early-departure terms, legal entity, corporate ownership record, complaints route, inspection results or named data controller is published. The conflicting statements on whether the founder medically directs care are not a fee, fee term, ownership or founding statement, so no contradiction deduction applies; two elements give band 3.
ConfidenceHow well the evidence supports the score.: medium
No court judgment, regulator decision or licence action against the operator was found within ten years. The 2016 Salon article concerns the group's Rosarito property and reports unresolved questions, not a finding, and falls outside the five-year window, so it scores nothing. No inspection record, first-hand bylined reporting from the Tulum premises or independent review base tied to this site exists; Trustpilot returns no profile and the homepage rating is not tied to a source. This gives band 5.
ConfidenceHow well the evidence supports the score.: low
The environment side meets one element: private oceanfront rooms with private bathrooms and on-site therapy infrastructure, giving a habitat sub-band of 2. A crash cart, defibrillator and transfer protocols are stated but no receiving hospital is named, and because the residence address is unpublished the emergency department travel time cannot be computed from where clients stay, so the hospital proximity element is not met. The nutrition side meets one element, nutrition in the discharge plan, but no nutrition professional is named, giving a sub-band of 3. The mean of 2 and 3 rounds up to band 3.
ConfidenceHow well the evidence supports the score.: low
The Medical Review Board page names a Medical Director with stated surgery and critical care titles but no registration number, and the founder is stated to hold no medical credentials. Lookups in the national professional licence register and the specialist certification register could not be completed and are queued, and the facility licence is not proven on a register. With no credential verified, no credential counts and the band is 1, provisional.
ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: Clinician register lookups for this criterion were still open at the edition cut-off (see known_gaps). The points count as assessed on the evidence available; the band may change when the lookups are completed, which is resolved in a later edition.
ConfidenceHow well the evidence supports the score.: medium
Three published tiers: the Gold Package at USD 300,000 for four to twelve weeks, the Platinum Package at USD 500,000 for four to twelve weeks in oceanfront accommodation, and the Elite VIP Executive Package at USD 1,000,000 and upward for twelve to sixteen weeks in a private beachfront mansion. At the stated durations that is roughly USD 25,000 to USD 75,000 per week. Private pay only, no insurance. Worth knowing before you compare: the same company's other property, running the same method under the same medical director, sells four weeks at around USD 50,000. No exclusions, deposit or refund terms are published.
Tulum, Quintana Roo, on Mexico's Caribbean coast, about two hours south of Cancun. The clinic describes a 10,000 square foot private oceanfront estate with pools, a yoga deck, a massage terrace, a gym and on-site medical infrastructure, and publishes no street address for it. The only address on the contact page is a business centre on Avenida Kukulkan, and third-party listings place the operation inland at Aldea Zama, roughly 3 km from the shoreline. Neither can be confirmed. Tulum has no tertiary hospital, and the nearest full cardiac and intensive-care capability is in Playa del Carmen or Cancun.
Ultra-high-net-worth individuals and executives seeking psychedelic-assisted treatment for opioid, alcohol, benzodiazepine or stimulant dependence, or for treatment-resistant depression, PTSD and trauma, who specifically want a model with no prescription medication, no group therapy and no conventional talk therapy. Three to five clients are on site at a time in private suites, with sole occupancy available at the upper tiers. It is not designed for anyone needing psychiatric medication management, CBT, DBT, EMDR or family therapy, none of which appear in the published programme.
Admission runs through a confidential review call that decides whether the model fits the person at all, followed by a pre-admission medical review in which the Medical Director reviews the cardiology workup, stress test, EKG, imaging and bloodwork and either clears the client or declines to. Screening before any ibogaine administration includes a full EKG, an echocardiogram and cardiac risk assessment, complete bloodwork with liver-function and metabolic panels, a medication interaction review, and supervised transition for anyone on benzodiazepines or opioids. Applicants with untreated cardiac conditions, severe liver disease, active suicidal intent or a medical contraindication to travel are declined and directed to acute care first. The founder states he is personally involved in every admission while making no clinical decisions. No waiting time is published.
Clients leave with a written integration plan covering sleep, nutrition, movement and ongoing supplementation, including structured check-ins with the clinical team, and the clinic says it stays reachable afterwards and helps arrange continued support locally where needed. No duration, no price, no defined follow-up schedule, no psychiatric follow-up, no alumni service and no outcome-tracking method is published, and whether aftercare is included in the fee is not stated. That is a significant gap at these prices and lengths of stay, and it should be settled in writing before committing.
