Ibiza,
One client to a villa, in Ibiza, Bali or Barbados, with a clinical and household team assembled for that person alone.
Istana publishes what almost nobody in this segment publishes: its price, in its own FAQ, at EUR 65,000 per week for Ibiza. It is a founder-led operator that admits one client per property, in a leased private villa, with the clinical, complementary and concierge team built around that individual and no groups, no shared spaces and no fixed programme. Three matched properties run in parallel in Ibiza, Bali and Barbados, so operator-wide capacity is up to three clients, one per location, which the operator states openly rather than obscuring. Founded in 2017 by Ian Ross-Smith, it treats addiction, trauma, depression, anxiety, grief, burnout and eating disorders, and extends to psychedelic-assisted psychotherapy. Confidentiality is run as an operational discipline with named, checkable controls. There is no health-centre licence in any of the three jurisdictions, and the operator describes itself as a private therapeutic retreat rather than a hospital or clinic, so the regulatory standing of the care rests on the individual practitioners' own professional registrations, none of which are published as numbers.
Four people are named with surnames on the operator's own about page, with stated professional memberships and degrees. Beyond them, the treating team is assembled per client and is not published in advance.

Pre-arrival and on arrival, variable
Included in the weekly fee
A preliminary clinical assessment by the clinical lead in the chosen location, typically by video call, before the booking is confirmed, followed on arrival by an initial medical assessment and bloodwork. The assessment determines whether detox is required and shapes the treating team around the case.
As clinically required
Included in the weekly fee
Managed by a dedicated medical team from the moment of arrival, with a doctor and nursing team present and 24-hour medical care for as long as the detox phase runs. It is delivered inside the villa rather than in a hospital or a shared detox unit, with no on-site hospital backup and no health-centre registration.
Continuous through the stay
Included in the weekly fee
Entirely one to one, covering trauma, PTSD, depression, anxiety, grief, stress, burnout and addiction. Specialists travel to the villa rather than the client attending sessions elsewhere. Sessions are held between client and therapist alone and reach the wider team only with the client's explicit consent. No session count or daily hours figure is published.

Offered for clients assessed as suitable, and delivered only where the operator states it can be legally and safely conducted, which it says is not at its standard locations. No protocol, no substance, no jurisdiction of delivery, no screening standard and no supervising physician is published for this modality.
Continuous through the stay
Included in the weekly fee
Trauma-informed yoga, breathwork, meditation, personal training, bodywork and holistic practice under the Head of Complementary Services, delivered one to one alongside the clinical work rather than as an optional extra. No practitioner other than the department head is named.
Continuous through the stay
Included in the weekly fee, excluding high-value activities
The reconnection element of the model: structured experiences and time outdoors, with staff dressed without uniforms on outings so the client is not identifiable. High-value recreation such as yacht charter and helicopter hire is excluded from the fee and billed separately.

Individually designed continuation of care after departure. Arrangements are organised before discharge, team members can travel home with the client and stay, and clinicians remain available by video, phone or in person. Neither the duration, the format, the delivery location, nor whether it is included in the residential fee is published.

The entire quantified public review base is two reviews on one platform, recorded under a brand name the operator no longer uses. There is no Google business profile with a public rating and no Trustpilot profile. The count is structurally explicable rather than suspicious: one client per property across three properties, selected for being high profile and bound into a confidentiality architecture, will not generate public reviews. No fabricated review pattern, no negative review and no complaint was found, and no documented breach or scandal was found in any source reviewed. The volume should be read against the Bloomberg coverage rather than as a reputation problem.

One tier-one mention. Bloomberg Businessweek, Mark Ellwood, "White-Collar Wipeout", April 2023, on the boom in luxury burnout retreats, with the founder quoted on the record that burnout admissions had risen 600 percent since March 2020. The piece is a trend article rather than an account of a facility visit, and the original could not be retrieved directly for a first-hand check. A Times Luxx Magazine item referenced on the homepage could not be located.

Istana operates no clinic building. It secures handpicked private villas, one per client per location, and states that it works with trusted owners and uses over 150 different villas which are regularly rotated. The operator's own minimum standard is published: all private residences have at least four bedrooms, a pool and private grounds. The Ibiza properties are described as discreet whitewashed hillside residences away from the tourist areas, in a peaceful rural setting, with sea-view options. Properties are unmarked and present as private estates: no signage, no clinical branding, no reception desk and no paperwork on arrival, because documentation is completed before the client travels. Vehicles are discreet and unbranded, and a vehicle with driver is included. Enhanced security is a paid upgrade. Not published: exact bedroom counts per property, square metreage, plot size, pool and gym specification, whether properties are owned or leased, or how many villas sit in the Ibiza rotation. The images the operator shows are described as examples of the standard of residences rather than as a specific property. No independent photography, press visit or third-party inspection exists, so the facility cannot be independently assessed.
A personal chef is a standing member of the support team and is explicitly included in the weekly fee alongside accommodation, clinical services, staff and vehicle. Dietary requirements are captured in the pre-arrival calls, before the client travels, and the operator states that nutritional requirements are assessed and a personalised plan prepared by the chef from the outset. The detox page adds that a nutritionist creates a diet to remedy any deficiencies and the personal chef brings it to life. Because one client occupies the villa, catering is individual by construction rather than a communal kitchen service. The evidence stops there. No chef is named in any of the three locations, and no training, restaurant background or culinary credential is published. No named nutritionist or dietitian with a stated qualification appears anywhere on the published team, and two operator pages describe who prepares the nutrition plan differently. No dietary testing, metabolic profiling, monitoring during the stay or hygiene certification is described.
no client contact
One client occupies each villa with a team assembled for that client alone, there is no group programme, and the operator states there are no shared facilities and no shared spaces. There is no shared therapy room, dining room, gym, pool, garden or reception in which an encounter could occur, because the whole property is the client's. Transport is a dedicated vehicle with driver. The three concurrent properties do not weaken this: each is a separate building in a separate country. The operator scopes the claim to each residence rather than overstating it across the operator. The supporting protocol set is among the most specific in the ranking. Legal: every team member signs a non-disclosure agreement on engagement, clients may supply their own agreements signed before arrival, and contracts are anonymised to the client's specification. Access control: only limited personnel know a client's full name, support staff use first names only, briefing is strictly need-to-know, and staff wear no uniforms on outings. Data: internal communications run on end-to-end encrypted applications, clients are given code names, all internal communications about a stay are erased on departure, anonymised email accounts and VPN set-up are offered, medical records are held by the medical team only, and clients receive full reports on departure and may request deletion. Physical: unmarked properties with no branding or signage, no reception desk, no paperwork on arrival, unbranded vehicles, and enhanced security on request. Every one of these controls is self-declared. No external standard is claimed, no third party has audited any of it, and because no regulator oversees the operator there is no body to which a confidentiality failure would be reported and no inspection record that would surface one. A clean record here is in part an absence of oversight.
Headline basis: Public evidenceThe headline number is the public score, not the extended score., LROAS 1.0.0, edition 2026-10-01
Public-evidence total: 44
The sum of the ten criterion scores, from 10 to 100, using evidence anyone can check, including the clinic's own pages as self-declared statements. For the licence element of C2 only, it can also use an authority ruling held in our sealed evidence room. It is the only number used for the rank.
ConfidenceHow well the evidence supports the score.: medium. ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: C10.
Scored on the occupancy model, not a bed count. The Ibiza page states one client at a time in a private residence staffed and secured for that individual; the FAQ states no shared facilities or spaces and a dedicated vehicle and driver; the confidentiality page names several mechanisms beyond statute (staff non-disclosure agreements, code names, encrypted communications erased on departure). No source shows two Ibiza residences occupied at once, so no contradiction deduction applies; the operator-wide Clinical Director being shared across locations does not bring two clients together and is considered under C4. Band 10 is not reached because no register records capacity per address and no independent first-hand report corroborates the separation.
ConfidenceHow well the evidence supports the score.: medium
No institutional licence was found: a name search of the Balearic health-centre register for Istana returned no results and the national register of health centres holds no Istana entry, and the operator describes itself as a private therapeutic retreat rather than a hospital or clinic. Withdrawal is managed in the residence by a doctor and nursing team, and psychiatry is stated in the fee inclusions, but 24-hour medical cover runs only until detox is completed, so round-the-clock physician cover is not established. No hospital route is named, no accreditation was found, and the operator-wide Clinical Director is presented as a psychologist, not a physician. Band 5 is not reachable without a licence.
ConfidenceHow well the evidence supports the score.: high
A preliminary clinical assessment by the local clinical lead, typically by video, takes place before confirmation, and an initial medical assessment with bloodwork follows on arrival. No imaging, ECG or neuropsychological testing is stated, the departure report is not described as a diagnostic formulation, and no psychiatrist or physician is named for Ibiza, so the elements for band 5 are not met.
ConfidenceHow well the evidence supports the score.: medium
A dedicated team per client is stated ('One client. One team.') and no clinical hours are delivered in groups. No session count or daily hours figure is published, and 24-hour medical presence in the residence is stated only for the detox phase, not for the whole stay. Without a published count, band 5 is out of reach.
ConfidenceHow well the evidence supports the score.: medium
Aftercare is organised before departure, the same clinicians remain available, team members or companions can return home with the client, and an extended stay in a supported private setting is offered. Aftercare is described as integral but is quoted and invoiced separately, so no inclusion or price is published. No duration, psychiatric follow-up or structured long-term contact is published. Four elements are met, which gives band 5.
ConfidenceHow well the evidence supports the score.: medium
One violation is recorded: the operator's own post 'The Best Luxury Rehab in the World: An Honest Guide' lists Istana first among eight named centres and answers its own FAQ question on the most private rehab with 'Istana'. The llms.txt file names competing clinics 'for context only' and neither ranks, recommends nor tells a model to prefer a clinic, so it is not counted. Positive signals: a published Ibiza weekly fee with no statement that it is negotiable or on application, a referral-out policy for clients needing hospital-level resources, and an operator-wide statement that the person's own agreement is needed before admission. One violation with three positive signals gives band 4.
ConfidenceHow well the evidence supports the score.: low
Met: fee inclusions and exclusions, payment terms (payment in full on booking, cleared funds seven working days before arrival, non-refundable except force majeure) and a statement of regulatory limits. Not met: no early-departure or extension term, no licence reference or inspection, the Singapore entity is named without registration number or registered address, ownership and a complaints route are not published, and the privacy policy names no controller or retention period. A 2024 archived FAQ carried a price range and a non-refundable deposit that are no longer live; a removed clause is not a current contradiction, and a EUR 70,000 figure above a 'from EUR 65,000' floor is not a conflicting fee, so no deduction applies.
ConfidenceHow well the evidence supports the score.: medium
No final regulator, court or tribunal finding was found for Istana Jiwa Pte Ltd, Istana Jiwa or Istana Private, including a Singapore eLitigation search with no judgments, and no inspection record exists. The Bloomberg Businessweek piece is a trend feature with no account of a visit and could not be retrieved for a first-hand check. Only one independent review platform carries reviews (two, under the former brand name) and no Trustpilot profile exists. This is the 'not published' band 5.
ConfidenceHow well the evidence supports the score.: low
Every residence has several bedrooms, a pool and private grounds, the fee includes a resident support team and security, and family visits are welcomed. Villas are rotated from a large pool and the images are examples, so the specific premises are not established; only the island is published, so the emergency department distance cannot be measured. No therapy room, emergency route or safety protocol is published, and the nutritionist is unnamed with no stated qualification.
ConfidenceHow well the evidence supports the score.: medium
The about page names four people, none specifically for the Ibiza site and none with a published registration number. Lookups for the operator-wide Clinical Director on the Balearic psychologists' register and the UK HCPC register returned no match, and that role is shared across locations and so not counted as core staff. No other treating clinician is named and the facility licence is not proven, so provisional verification cannot apply.
ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: Clinician register lookups for this criterion were still open at the edition cut-off (see known_gaps). The points count as assessed on the evidence available; the band may change when the lookups are completed, which is resolved in a later edition.
ConfidenceHow well the evidence supports the score.: medium
Istana publishes its fees openly in its own FAQ, which is unusual at this level. Ibiza is EUR 65,000 per week, Barbados USD 65,000 and Bali USD 40,000. The fee covers clinical services, detox nursing, accommodation, meals, the assembled staff team, a vehicle with driver and 24-hour concierge. Excluded are international travel, cosmetic and elective procedures, dentistry, unrelated medication and scans, and high-value recreation such as yacht charter. Everything is private pay: the operator states plainly that its fees are not covered by health insurance, and full payment is required before a booking is confirmed.
On Ibiza, in the Balearic Islands, at an address the operator deliberately withholds. The properties are described as discreet whitewashed hillside residences set away from the tourist areas in a rural setting, with sea-view options, presenting as private estates with no signage, branding or reception. Ibiza is positioned a few hours' flight from London, Paris and Zurich. Two matched single-client properties run in parallel in Bali and Barbados, the latter eight hours from London. No property can be independently located or photographed.
High-profile and ultra-high-net-worth individuals for whom reputation preservation matters as much as clinical outcome: executives, public figures and members of wealthy families. Istana treats alcohol and substance dependence, trauma and PTSD, depression, anxiety, grief, stress, burnout, eating disorders, gambling and sex addiction. Everything is delivered one to one, with no groups and no shared facilities. It is not for anyone needing acute hospital-level stabilisation or psychiatric detention, since the operator describes itself as a therapeutic retreat rather than a hospital or clinic.
Contact is by the site's message form or WhatsApp, and the founder conducts the introductory consultation personally. A preliminary clinical assessment by the clinical lead in the chosen location follows, typically by video call, before the booking is confirmed. The operator states that admission is typically seven to ten days from first contact, that a villa and team can be assembled inside 48 hours where the situation is urgent, and that a medical team can reach a client within two hours in a crisis. Bookings are confirmed only on receipt of full payment. Contracts are anonymised to the client's specification and all paperwork is completed before travel.
Aftercare arrangements are organised before departure, team members can travel home with the client and stay, and the clinicians the client has come to trust remain available by video, by phone or in person. Clients receive full clinical reports on discharge and may request deletion of their records. This is the least-documented part of the proposition: the duration, the session format, the delivery location, and whether aftercare is included in the residential fee or billed separately are all unpublished. Anyone weighing Istana against operators that publish fixed aftercare contracts should get the aftercare terms in writing before committing.
