Sundance,
Twelve-step clinical treatment across two Utah campuses, a Wasatch timber lodge and a converted television studio.
Cirque Lodge is a founder-owned Utah addiction treatment centre built on a twelve-step clinical spine and an experiential therapy programme that is unusually well resourced. It runs two campuses and one programme. The Lodge at Sundance takes up to 19 clients in a timber mountain house in Provo Canyon. The Studio in Orem takes up to 44 in the former Osmond television studios, which carry the heavy infrastructure the mountain site cannot: indoor and outdoor horse arenas, a 17,000 square foot indoor ropes and challenge course, an art therapy room, a recording studio and organic gardens. Treatment combines individual and group psychotherapy, medically supervised detoxification and family work with hiking, rafting, equine work and art. It opened in 1999 and has run continuously since, with several senior staff in post for two decades or more. It is licensed by the State of Utah as a residential substance use disorder programme rather than as a hospital, and no state register entry or accreditor register entry could be retrieved to confirm the licence or the accreditation independently. No lawsuit, state enforcement action or licence restriction against the operator was found.
The clinic publishes a full staff roster with photographs and credentials: 45 named people across both campuses, of whom 21 hold a claimed clinical credential and a clinical role at this site.

Typically 3 to 7 days, as the entry stage of residential treatment
-
Medically supervised withdrawal management delivered at the Orem campus, with first-floor medical examination rooms, a detox lounge and a nurse's station with monitoring systems alongside it, and round-the-clock monitoring during detoxification. It covers alcohol, opioid, benzodiazepine and stimulant withdrawal. There is no psychiatric hospital unit behind it, so clients who destabilise beyond a residential level of care must be transferred out, and no escalation hospital is named.
30 days standard, extendable to 90
-
Up to 19 clients in a timber mountain house in Provo Canyon, in private or shared bedrooms with private bathrooms, jetted tubs and fireplaces or wood stoves. Therapy, group work, a circular meditation room, exercise equipment and daily mountain recreation are all on site. A private bedroom is available at increased cost, and the clinic recommends shared rooms.
30 days standard, extendable to 90
-
The larger campus and the functionally richer one. Up to 44 clients on gender-separated floors in the converted Osmond television studios, with the equipment the mountain site cannot carry: horse arenas, the indoor challenge course, an art therapy room, a recording studio, a fitness centre and organic gardens. Detoxification and the Level 7 step-down are also based here.
Integrated throughout the stay
Included in the residential programme
The clinic's signature and the reason the Orem campus exists in the form it does. Equine work, a 17,000 square foot indoor ropes and challenge course, art and music therapy including studio recording, and hiking, rafting and fishing in the Wasatch, used to surface material that talk therapy alone does not reach.
30 to 90 days, set by clinical assessment and progress
Not published separately
A dedicated track for physicians, nurses and other licensed clinicians facing board or licensure consequences, coordinated by a nurse-qualified programme lead. It publishes a minimum of one individual counselling session a week with a licensed counsellor and one individual case-management session a week, weekly alumni and aftercare support groups, lifetime guesting privileges, and work with employers and licensing agencies on return-to-work compliance.
One week within the residential stay, held every three weeks
Included in the residential programme
A structured multi-day family workshop with education, multi-systemic family group therapy, individual family sessions and guest lectures. After the first 72 hours families may take part in weekly face-to-face or telephone sessions. The premise is that the household a client returns to should be worked with directly rather than briefed afterwards.
-
-
Apartment-style step-down at the Orem campus, described as a bridge between residential treatment and long-term independence, open to clients who have completed at least 90 days of residential treatment. It publishes a minimum of one individual counselling session, one case-management session and three process groups a week, with a daily on-site medical provider, nursing cover around the clock and psychiatry consultations as needed.
Ongoing after discharge
-
A personalised aftercare plan written before discharge, then alumni retreats and events, a three to four day guesting privilege, a community app and a two-week relapse track. Clients stepping down are referred into outpatient and twelve-step support in their home area. No duration, cost, clinician or telehealth detail is published.
No public price found.
The verifiable review base is thin. Roughly twenty-six identifiable reviews across two platforms after twenty-seven years of operation, with no Google rating or count that could be established for either campus and no Trustpilot listing. The ratings that exist are good and there is no sign of manipulation in either direction. The most plausible reading is a clinic that does not solicit reviews, which is consistent with its privacy posture and scores badly on a rubric that counts them. The clinic's own testimonials page carries more than thirty unattributed quotes and links to no independent platform, so it is not testimony. The litigation and regulatory record deserves saying plainly: no lawsuit, state enforcement action, licence restriction, civil penalty or facility-caused confidentiality breach against the operator was found. For a United States residential provider with twenty-seven years of continuous operation and sixty-three beds, that is uncommon and it is the strongest reputational fact in this file. It is not a positive register finding, though: the state compliance history could not be retrieved at all.

A national business magazine profiled the clinic with facility access in October 2009 under a headline about rehab for the rich and famous; direct retrieval returned an error, so its content is taken from the indexed summary and is marked partially verified. Other coverage includes a national newspaper in 2008, a national broadcaster in 2007, a state newspaper in 2003 and a local newspaper in February 2020 on the Orem expansion. No first-hand bylined reporting from inside the facility within the last five years was found. The clinic publishes no press page and no media kit.
No industry award was located. The clinic's about page reports a professional association career achievement award to its emeritus director, which is an award to a person rather than an accreditation of the facility.
The Lodge, Sundance. A timbered mountain retreat in Provo Canyon beside Sundance Mountain Resort, taking up to 19 clients in private or shared bedrooms. Rooms have large private bathrooms with jetted tubs, down duvets, reading areas and fireplaces or wood-burning stoves, with large windows onto waterfalls and year-round snowfields. On site there are individual counselling and group therapy rooms, exercise equipment and a circular meditation room. A smaller adjacent building, Lookout, holds three further places; whether it remains in use is not published. The Studio, Orem. The former Osmond Studios, converted and expanded, with a further expansion reported in February 2020. Up to 44 clients in private or shared bedrooms on separate floors for men and women, with gender-separated group sessions. Twelve acres carry indoor and outdoor horse arenas, a 17,000 square foot indoor rope and challenge course, an art therapy room, a fitness centre, a recording studio, hybrid rose and organic vegetable gardens, volleyball, pickleball and bocce courts, an outdoor movie theatre, a labyrinth, a half-mile walking path and a twelve-step walking path. Medical examination rooms, a detox lounge and a nurse's station are on the first floor. The two properties are not equivalent. The Lodge is the scenically superior building and the Studio is the functionally richer one, a converted industrial television facility in a valley town rather than a mountain setting. Clients are placed at one or the other by clinical fit and accommodation preference, which is a real difference in product for someone who is not told which they will get before the admissions call.
Three well-balanced meals a day, using ingredients the clinic describes as fresh and locally sourced where possible, with a kitchen stocked with fruit, vegetables and snacks available at any hour. Individualised meals are prepared on request, and special diets are accommodated with advance notice. The clinic frames nutrition clinically, noting that addiction causes physical damage and nutritional deficiency and that balanced meals affect mood. The Studio grows organic vegetables in its own gardens. No chef is named anywhere on the clinic's site, and no nutritionist or dietitian appears on the published staff roster. No nutrition assessment on admission, no nutrition protocol and no monitoring during the stay is published. On the record available this is competent institutional catering with dietary accommodation, and nothing in the clinic's own material claims otherwise.
one to one therapy
Individual psychotherapy runs throughout the stay alongside group work, so one-to-one therapy is established. Nothing beyond that is. Both campuses use shared as well as private rooms, and both house clients in a single building. Up to 63 clients are in treatment across the two campuses with group therapy as the clinical spine, gender-separated group floors, communal dining, Family Week every three weeks with multi-family group sessions, and guest lecturers. Clients see each other continuously. This is the structural cap on the score and it is a design choice, not a lapse. The clinic names one confidentiality mechanism beyond statute: a private gated entrance serving residents and their families. Records confidentiality is a statutory duty rather than a clinic measure. No NDA policy, no separated-arrival protocol, no scheduling that keeps clients apart, no phone or photography policy and no anonymity guarantee is published. No confidentiality breach caused by the facility was found. What is on the record, and it is not a breach, is that several public figures have been reported in the press as having been treated here, in most cases through their own or their representatives' disclosures rather than through the facility. Their names are withheld here. Separately, the clinic's own director of operations told a national magazine in 2009 that a notable share of the client base would be recognised immediately. That breaches nobody's confidentiality, but it is the clinic treating the profile of its clientele as reputational currency, and it sits awkwardly beside a discretion-first standard.
Headline basis: Public evidenceThe headline number is the public score, not the extended score., LROAS 1.0.0, edition 2026-10-01
Public-evidence total: 37
The sum of the ten criterion scores, from 10 to 100, using evidence anyone can check, including the clinic's own pages as self-declared statements. For the licence element of C2 only, it can also use an authority ruling held in our sealed evidence room. It is the only number used for the rank.
ConfidenceHow well the evidence supports the score.: low. ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: C10.
Band 2 applies because shared bedrooms are offered at any price tier and group work is a core part of the programme: the admissions page recommends shared rooms and sells a private room only at extra cost, and Family Week multi-family group therapy and lectures are central. A private bedroom does not amount to single occupancy of a residence, so no uplift to a higher band applies, and no exclusive-use arrangement was established. No contradiction deduction applies because the clinic makes no one-client claim. One confidentiality mechanism beyond statute is named, a private gated entrance.
ConfidenceHow well the evidence supports the score.: low
Band 6 applies: the Joint Commission accredited-organisations list records the operator as accredited in Behavioral Health Care and Human Services at both the Sundance and Orem sites (effective 15 March 2025), which meets the register condition, and a physician Medical Director and a psychiatrist are named on the staff page. Two further conditions are met: medically supervised withdrawal on site at the Studio, and current accreditation of the clinical service shown on the accreditor's own register. Band 7 needs three further conditions, and the clinic does not name physician or psychiatrist cover overnight (withdrawal is described as round-the-clock supervision without naming the profession), names no escalation hospital, offers no non-psychiatric physician service and publishes no defined programme for complex psychiatric conditions.
ConfidenceHow well the evidence supports the score.: medium
Band 2 applies: before commitment only an intake call or online form is published, and the assessor is not described as a clinician. On arrival a history and physical examination is completed by medical staff, not stated to be a physician. No psychiatric assessment, laboratory work, on-site diagnostics or written formulation is published, so no higher band is reached.
ConfidenceHow well the evidence supports the score.: medium
Band 3 applies: no count of individual sessions is published for the standard residential programme. The healthcare professionals track and the Level 7 step-down publish a minimum of one individual counselling session a week, and the clinical team is shared across both campuses with no staff-to-client ratio published. The stay length is set clinically rather than sold as a fixed package, so no cap applies.
ConfidenceHow well the evidence supports the score.: low
Band 5 applies with four aftercare elements met: a personalised aftercare plan is written before discharge, and the operator runs its own Level 7 step-down at the Orem campus with a daily medical provider, round-the-clock nursing and psychiatry consultations as needed. The alumni offer (retreats, events partly held online, an app, a guesting privilege and a relapse track) is peer and social support with no published duration, price or clinician delivery, so it does not count as clinical aftercare and no further element is added.
ConfidenceHow well the evidence supports the score.: medium
Band 4 applies: a clean sweep with no positive signal. No clinic page ranks or lists other clinics, no content directed at AI assistants was found, and no paid recognition is presented as independent (the professional association award is shown as an award to a person). The admissions page lists people the clinic will not admit but says nothing about referring them elsewhere, and no price, referral-fee position or voluntary-admission policy is published, so none of the positive signals needed for band 5 is met.
ConfidenceHow well the evidence supports the score.: low
Band 4 applies with three transparency elements met: fee exclusions are itemised on the admissions page, a HIPAA privacy notice states a retention period of at least seven years, and the admissions page lists conditions the clinic does not treat. Refund and cancellation terms, extension or early-departure terms, the legal entity, ownership, a licence number or register link, inspection results and an external complaints route are not published by the clinic, so no higher band is reached. The accreditor record carries an organisation number, but the clinic does not publish it itself.
ConfidenceHow well the evidence supports the score.: medium
Band 5 applies: searches of CourtListener, Utah appellate opinions and the web found no final regulator, court or tribunal outcome against the operator within ten years, and no institution-level allegation. No statutory inspection result is on record (accreditation is not a statutory inspection), and there is neither recent first-hand independent reporting nor review evidence on two independent platforms, so no higher band applies. The clean record is an absence of findings, not a positive register result.
ConfidenceHow well the evidence supports the score.: low
Band 4 is the average of two sub-scores: environment and safety 5, nutrition 3. Environment and safety credits clinical and medical rooms, outdoor and exercise space on both campuses, family visit arrangements, a published medication and substance policy (abstinence-based, controlled substances destroyed on admission, prescribed medication dispensed through a pharmacy) and premises confirmed on the accreditor's register; it stops at 5 because no emergency department travel time was verified. Nutrition reaches 3 on dietary accommodation only, with no named dietitian or meal planner.
ConfidenceHow well the evidence supports the score.: high
Band 2 applies: isolated registered professionals. The Utah licensee lookup on 2026-09-16 confirmed three credentials: active physician and surgeon licences for the Medical Director and the psychiatrist, and an active clinical social worker licence for the Clinical Director. No specialty is recorded for either physician on the register, the clinic publishes no role-class credential statement, and the remaining named claims are unchecked, so no gate for a higher band is met.
ProvisionalA check was still open when the edition closed, for example a register lookup or a video not yet received, or evidence under embargo.: Clinician register lookups for this criterion were still open at the edition cut-off (see known_gaps). The points count as assessed on the evidence available; the band may change when the lookups are completed, which is resolved in a later edition.
ConfidenceHow well the evidence supports the score.: medium
The clinic publishes no price anywhere on its site, and cost is handled at the admissions call. Third-party figures vary by roughly a factor of two and a half and none of them is confirmed against a primary source, so no price is recorded here. Payment is due in full on admission by card, cheque or wire, and a card is held on file for incidental charges including medications through a third-party pharmacy, physician visits, co-pays, books and salon services. The clinic is out of network and not contracted with government payers. Scholarships are reported to exist for clients who cannot afford treatment.
Two campuses in Utah County. The Lodge is at 3114 E Ida's Road, Sundance, in Provo Canyon beside Sundance Mountain Resort. The Studio is at 777 N Palisade Drive, Orem, about 25 minutes away in the valley. Salt Lake City International Airport is roughly 45 to 60 minutes north by road, and Provo Airport is closer for private aviation. Staff arrange airport pickup. Clients are placed at one campus or the other according to clinical fit and accommodation preference.
Adults with substance use disorders and co-occurring conditions who want a twelve-step clinical foundation combined with serious outdoor and experiential work, and who can pay privately or use out-of-network benefits. A dedicated Healthcare Professionals Programme serves physicians, nurses and other licensed clinicians facing board or licensure consequences. It is a group-based campus model, so it does not suit anyone who requires that no other client sees them. The clinic states it will not admit people whose psychiatric condition needs acute hospitalisation, whose physical limitations prevent full participation, whose behaviour is incompatible with group living, who cannot understand English, or who are under 18.
Admission runs through an admissions line, where a member of staff takes the caller through a confidential assessment to establish what kind of programme is needed and which campus fits. The assessor is not described as a clinician and the assessment is not described as psychiatric, medical or psychological. On arrival a history and physical examination is completed by medical staff. Because the clinic quotes individually rather than by list price, the cost conversation and any out-of-network insurance verification happen at this stage. No waiting time is published.
Medically trained professionals create a personalised aftercare plan before discharge. An alumni programme offers weekly alumni and aftercare support groups, alumni retreats and events, lifetime guesting privileges of three to four days, a community app and a two-week relapse track. No duration, price or inclusion statement is published for any of it, and the pages do not say clinicians deliver it. A Level 7 apartment-style step-down at the Orem campus is open to clients who have completed at least 90 days of residential treatment, with a daily on-site medical provider, 24-hour nursing and psychiatry consultations as needed. The clinic runs no outpatient or virtual step-down network of its own beyond that.
